J O I N T   R E S O L U T I O N

RESPECTFULLY REQUESTING THE RHODE ISLAND DEPARTMENT OF HEALTH TO PROVIDE ADDITIONAL INFORMATION REGARDING LICENSED ASSISTED RESIDENCES ON ITS WEBSITE

WHEREAS, Consumers and their families have a need to make informed choices in selecting long-term care supports and services, and assisted living residences may be an appropriate long-term option for those seeing such services; and

 WHEREAS, It is important that the state provide online access to basic information about licensed assisted living residences in the state; now, therefore be it

 RESOLVED, That this General Assembly of the State of Rhode Island and Providence Plantations hereby respectfully requests the Department of Health to provide the following information regarding licensed assisted residence on its website:

1. The name of the assisted living residence;

2. The address, phone number, and email address if available;

3. The number of units;

4. The Level of Licensure;

5. The name of the current administrator and/or key contact person of record;

6. Whether the residence accepts government payment assistance programs such as Medicaid and SSI;

7. Links to the latest annual inspection report; and

8. Links to the required disclosures for any Special Care dementia licenses and Limited Health Services licensees; and be it further

RESOLVED, That the Secretary of State be and hereby is authorized and directed to transmit a duly certified copy of this resolution to the Director of the Rhode Island Department of Health.
