§ 28-43-6. Notification of benefit payments to employers.

(a) Within thirty (30) days after the end of each calendar quarter, or more frequently if deemed necessary, the director shall furnish to each employer who or that is charged for benefits paid to claimants under this title during that quarter a notice that shall contain at least the following information:

(1) The name and social security number of the payee;

(2) The amount paid;

(3) The date on which that payment was made;

(4) The calendar week for which that payment is made; and

(5) The name of the chargeable employer and the employer’s registered account number.

(b) That notification to an employer shall constitute the director’s determination and shall be binding upon the employer unless an appeal is duly filed within fifteen (15) days of its mailing.
